Role Overview
We are seeking an experienced and strategic Director of Case Management to lead our case management initiatives and enhance the quality of care provided to our clients. In this pivotal role, you will oversee the development and implementation of case management programs, ensuring that they align with our organizational goals and regulatory requirements.
Responsibilities
- Develop and execute a comprehensive case management strategy that drives measurable outcomes and improves client satisfaction.
- Lead a team of case managers, providing mentorship, guidance, and performance evaluations to foster professional growth and enhance team effectiveness.
- Collaborate with cross-functional teams, including clinical, operational, and administrative departments, to ensure seamless service delivery and coordinated care.
- Monitor case management processes and performance metrics, utilizing data analytics to identify opportunities for improvement and implement best practices.
- Ensure compliance with all relevant regulations, policies, and standards for case management practices.
- Act as a key liaison with external stakeholders, including healthcare providers and community organizations, to enhance service coordination and resource utilization.
Required and Preferred Qualifications
Required:
- Master's degree in Social Work, Nursing, Public Health, or a related field.
- 7+ years of experience in case management, with a minimum of 3 years in a leadership role.
- Strong knowledge of healthcare regulations, case management standards, and best practices.
- Proven ability to develop and implement effective case management programs that deliver measurable results.
Preferred:
- Experience with electronic health records (EHR) systems and case management software.
- Certification in case management (CCM, ACM, etc.) is highly desirable.
- Demonstrated experience in managing multidisciplinary teams and driving change initiatives.
Technical Skills and Relevant Technologies
- Advanced proficiency in data analysis tools and methodologies to inform decision-making and program development.
- Experience with health information exchange (HIE) and population health management systems.
- Familiarity with telehealth platforms and their integration into case management processes.
