Role Overview
We are looking for a dedicated and compassionate Assistant Case Manager to join our team. In this role, you will support case management services by assisting with the assessment, planning, and implementation of client care plans, ensuring that our clients receive the highest quality of service.
Responsibilities
- Assist in the development and implementation of individualized care plans for clients based on their specific needs.
- Conduct thorough assessments and evaluations to determine client eligibility and needs.
- Maintain accurate and up-to-date client records, documenting all interactions and interventions.
- Facilitate communication between clients, families, and service providers to ensure continuity of care.
- Monitor client progress and provide support, guidance, and resources as needed.
- Participate in team meetings and case reviews, contributing insights and recommendations for client care.
Required and Preferred Qualifications
Required:
- Bachelor's degree in Social Work, Psychology, or a related field.
- 1+ years of experience in case management, social services, or a related role.
- Strong organizational and time management skills, with the ability to manage multiple cases simultaneously.
Preferred:
- Experience working with diverse populations and understanding of cultural competency.
- Familiarity with local resources and services available to clients.
Technical Skills and Relevant Technologies
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint) and case management software.
- Experience with electronic health records (EHR) systems.
Soft Skills and Cultural Fit
- Excellent verbal and written communication skills.
- Empathetic and compassionate approach to client care.
- Ability to work independently and as part of a collaborative team.
- Strong problem-solving skills and the ability to think critically.
