Role Overview
We are seeking a highly organized and compassionate Assistant Care Manager to support our care management team in delivering exceptional services to our clients. In this role, you will assist in coordinating care plans, ensuring effective communication between clients, families, and healthcare providers, and contributing to the overall improvement of our care management processes.
Responsibilities
- Assist in the development and implementation of individualized care plans for clients, ensuring their needs and preferences are met
- Coordinate communication between clients, families, and healthcare providers to facilitate seamless care transitions
- Monitor client progress and outcomes, documenting any changes and escalating concerns to the Care Manager as needed
- Provide support in assessing client needs through interviews and evaluations, utilizing care management tools and resources
- Participate in regular team meetings to discuss client updates, share insights, and collaborate on care strategies
- Maintain accurate and up-to-date client records in compliance with regulatory standards and organizational policies
Required and Preferred Qualifications
Required:
- Associate’s degree in a relevant field or equivalent experience in healthcare or social services
- Strong interpersonal and communication skills, with the ability to build rapport with clients and their families
- Demonstrated organizational skills and attention to detail in managing multiple tasks
- Basic understanding of care management principles and practices
Preferred:
- Bachelor’s degree in social work, nursing, or a related field
- Experience in a care management or healthcare setting
- Knowledge of community resources and healthcare systems
- Proficiency with electronic health record (EHR) systems
Technical Skills and Relevant Technologies
- Familiarity with care management software and tools
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook)
- Basic understanding of data entry and documentation practices in healthcare environments
