Role Overview
We are seeking a dedicated and experienced Case Management Supervisor to lead our team in providing exceptional case management services. In this role, you will be responsible for overseeing case managers, ensuring high-quality care for clients, and fostering a collaborative environment that promotes professional growth and effective service delivery.
Responsibilities
- Supervise and support case management staff, providing guidance and mentorship to enhance their skills and performance.
- Monitor case management processes, ensuring compliance with regulatory standards and organizational policies.

- Collaborate with interdisciplinary teams to develop comprehensive care plans tailored to client needs.

- Implement training programs for staff on best practices in case management and client engagement.

Required and Preferred Qualifications
Required:
- Bachelor's degree in Social Work, Psychology, Nursing, or a related field.
- 5+ years of experience in case management or a related field, including supervisory experience.
- Strong understanding of case management principles, techniques, and methodologies.
- Excellent communication and interpersonal skills, with the ability to build rapport with clients and team members.
- Knowledge of relevant laws, regulations, and ethical standards in case management.
Preferred:
- Master's degree in a related field.
- Experience in a healthcare or social services setting.
- Certification in case management (e.g., CCM, ACM).
Technical Skills and Relevant Technologies
- Proficient in case management software and electronic health records (EHR) systems.
- Ability to analyze client data and generate reports to inform decision-making.
- Familiarity with data management tools and quality improvement methodologies.
