Role Overview
We are seeking a dedicated and experienced Patient Care Manager to join our healthcare team. In this critical role, you will oversee patient care activities, ensuring high-quality service delivery and advocating for patient needs. You will collaborate with multidisciplinary teams to develop and implement patient care strategies that enhance overall patient satisfaction and health outcomes.
Responsibilities
- Lead and manage daily operations of the patient care team, ensuring compliance with healthcare regulations and standards
- Develop, implement, and evaluate patient care plans in collaboration with healthcare professionals
- Monitor patient progress and adjust care plans as necessary to meet changing patient needs
- Serve as a primary point of contact for patient inquiries, concerns, and feedback, fostering a patient-centered culture
- Conduct staff training and performance evaluations, promoting continuous professional development and skill enhancement
- Analyze patient care data to identify trends and implement improvement initiatives to enhance patient experience
- Coordinate with external healthcare providers to ensure seamless care transitions for patients
Required and Preferred Qualifications
Required:
- Bachelor's degree in Nursing, Healthcare Administration, or a related field
- 5+ years of experience in a healthcare setting, with at least 2 years in a supervisory or managerial role
- Current nursing license (if applicable) and relevant certifications in healthcare management
- Strong understanding of patient care standards and regulations
Preferred:
- Master's degree in Nursing, Healthcare Administration, or related field
- Experience with electronic health records (EHR) systems and data management
- Proven ability to implement quality improvement initiatives in a healthcare environment
Technical Skills and Relevant Technologies
