Role Overview
We are seeking a strategic and experienced Director of Case Management to lead our case management operations, ensuring the delivery of high-quality services that meet the needs of our clients. In this role, you will oversee the development and implementation of case management strategies, collaborate with cross-functional teams, and drive the continuous improvement of processes to enhance client outcomes.
Responsibilities
- Lead and manage the case management team, providing guidance, support, and mentorship to ensure the delivery of effective and efficient services.
- Develop and implement case management policies, procedures, and best practices to enhance service delivery.
- Collaborate with clinical and operational teams to ensure a holistic approach to client care and support.
- Monitor and analyze case management metrics to assess program effectiveness and identify areas for improvement.
- Ensure compliance with regulatory requirements and industry standards in all case management activities.
- Foster relationships with community resources and stakeholders to enhance service availability and client support.
Required and Preferred Qualifications
Required:
- Bachelor’s degree in Social Work, Nursing, or a related field; Master’s degree preferred.
- 5+ years of experience in case management or a similar role, with a proven track record of leadership and program development.
- Strong knowledge of case management principles, best practices, and regulatory requirements.
- Excellent communication, interpersonal, and organizational skills.
Preferred:
- Experience in a healthcare or social services setting.
- Familiarity with electronic health records (EHR) and case management software.
- Relevant certifications such as Certified Case Manager (CCM) or Accredited Case Manager (ACM).
Technical Skills and Relevant Technologies
