Role Overview
We are looking for a dedicated and compassionate Junior Nurse Case Manager to join our healthcare team. In this fully remote position, you will play a vital role in coordinating patient care, ensuring continuity of services, and advocating for patients' needs throughout the healthcare system.
Responsibilities
- Assist in developing and implementing individualized patient care plans, collaborating with healthcare providers, patients, and families.
- Conduct comprehensive assessments of patients' needs and preferences to facilitate appropriate care interventions.
- Monitor patients' progress and adjust care plans as necessary, ensuring alignment with their healthcare goals.
- Educate patients and families on treatment plans, medication management, and available resources within the healthcare system.
- Document patient interactions and maintain accurate, up-to-date records in compliance with regulatory standards.
- Participate in interdisciplinary team meetings to ensure cohesive care coordination.
Required Qualifications
- Active Registered Nurse (RN) license and a minimum of an Associate's degree in Nursing.
- 1+ years of clinical nursing experience, preferably in a case management or related field.
- Strong understanding of healthcare systems, patient advocacy, and care coordination strategies.
- Excellent communication skills, both verbal and written, with the ability to convey complex information clearly.
- Proficiency in using electronic health records (EHR) and case management software.
Preferred Qualifications
- Bachelor's degree in Nursing (BSN) or related field.
- Certification in case management (CCM) or willingness to obtain certification within the first year of employment.
- Experience working with diverse patient populations and understanding of cultural competency in healthcare.
Technical Skills and Relevant Technologies
- Familiarity with electronic medical records (EMR) systems and telehealth platforms.
- Understanding of healthcare regulations and compliance standards.
- Knowledge of community resources and referral processes for patients.
