Referrals and Intake Coordinator

SUMMARY
The Referral Intake Coordinator assists clients in making the first step towards their better future. Is part of a centralized and growing team of professionals that are the first point of contact to our referral sources including but not limited to; individuals, parents, and healthcare providers seeking services for Behavior Health and Early Childhood Development. This role focuses on coordinating the collection of pertinent information to match the client with the relevant program(s) to fit their needs.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.
• Provide compassionate customer service and establish / maintain relationships with referral sources
• Provide knowledgeable information regarding company services including programs and product lines offered
• Answer calls and process email, fax, website and internal inquiries in a timely and professional manner
• Provide information to referral source inquiries regarding programs and services offered
• Gather and enter necessary data in the intake system (Credible) including demographic, insurance and desired service information
• Ensure supporting documentation is entered into the Electronic Health Record
• Providing accurate information to person served including scheduling an initial appointment or providing information regarding next steps
• Facilitate determination of referral viability
• Establish and maintain knowledge and proficiency of health plans in order to perform health insurance eligibility and verification to ensure proper handling and placement of each referral
• Verify benefits for health insurance using portals (EVS, Tuffs, Availity, Optum, WellCare, etc.)
• Relay accurate information regarding coverage to person served
• Identify all types of funding possibilities including primary, secondary, and ancillary
• Responsible for caseload completion including follow up on incomplete cases
• Routinely run and analyze reports in regards to productivity
• Collaborate and communicate issues and concerns with all internal and external staff as necessary
• Assist with special projects as department growth occurs within additional markets
• Performs other related duties and activities as required.

Minimum Knowledge and Skills required for the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.
Education and Experience:
• High School diploma or equivalent; Associate or Bachelor’s degree preferred
• 1+ year Customer Service experience with strong communication and analytical skills
• Healthcare experience (i.e., Patient Access, Admission) preferred. Behavioral Health experience is a plus
Certificates, Licenses, and Registrations:
• N/A
Other Skills and Abilities:
• Desire and ability to work independently in a private home setting to achieve performance goals
• Detail-oriented with ability to multi-task in a fast paced environment
• PC literacy and experience with MS Office Applications

Physical Requirements:
• Normal Office Work, Data Entry: Sedentary work. Exerting up to 10 pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
