Serves as designated ministry liaison with providers and ministry Care Coordination teams, to move patients towards safe and effective discharge plans or transitions to the most appropriate next level of care.

Required qualifications:
- Associate's Degree in Nursing
- Bachelor's Degree in Nursing. Or
- Upon hire: California Registered Nurse License.
- 3 years Experience in Utilization Management.
- Experience working with InterQual and MCG guidelines.
Preferred qualifications:
- Master's Degree in Nursing.
- 5 years Experience as a utilization/case manager in an acute care setting.
- Experience in a multi-hospital and/or integrated healthcare system.
