We are offering our Residential Habilitation Specialist position as a full-time position located throughout Essex County, NY.
What you get to do:
Actively participate in the planning and provision of all support services for individuals served by Mountain Lake Service, including day habilitation, residential habilitation, at home residential habilitation, job coaching and respite. Ensure corporate compliance and accurate documentation of services provided. Liaison with team members and attend all planning and treatment meetings to share information as needed.
Maintain daily documentation of services and completing other required records in line with OPWDD regulations and agency policies is essential. Prepares comprehensive monthly summary notes for assigned individuals in compliance with OPWDD regulations and agency policies.
Develop meaningful in-house and community activities, with an emphasis on activities of choice and social experiences. Present an acceptable role model in areas of communication, socialization, appearance and activities of daily living.
Requirements:
Minimum of a Bachelor's Degree in Sociology, Psychology, Social Work, or related field required.
Previous experience and training is desired but not necessary.
