Through groundbreaking technology, we are revolutionizing healthcare at Belong Health by focusing on patient-centered solutions and reducing the cost of care for underserved and complex populations. Our innovative approach involves partnering with regional health plans and hospital systems to launch and manage Medicare Advantage (MA) and Dual-Eligible Special Needs Plan (D-SNP) insurance offerings. We are not simply consultants; we act as a risk-bearing partner, sharing in the financial risk and providing long-term support to our collaborators. This unique partnership model allows us to develop custom solutions tailored to the specific needs of each community and its members. Our comprehensive, full-stack operating platform integrates seamlessly with existing offerings, filling gaps and adding strengths to provide holistic support.

At the core of our mission is a commitment to advancing health equity. We deploy in-market care teams that work in tandem with a partner's existing care infrastructure and community-based resources. These teams leverage Belong Health's proprietary 'Belong Connect' platform, which utilizes behavioral science and psychology to optimize care management and delivery. This technology enables us to tailor the sequence, cadence, and composition of care directly to each member's unique needs, fostering better engagement and outcomes. We are dedicated to making high-quality healthcare accessible to all, particularly those individuals with complex needs who have historically been hard to reach. By focusing on a whole-person model of care that includes medical, behavioral health, substance abuse, and social services coordination, we aim to deliver a more compassionate and straightforward healthcare experience. The recent formation of Belong Medical Group further enhances our ability to address chronic gaps in patient care by providing virtual and community-based resources.
