Our myPlace PACE (Program of All-Inclusive Care for the Elderly) model provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services – all at no cost if you are eligible for Medi-Cal or both Medicare and Medi-Cal.

At myPlace Health, we aim to deliver tailored care that enables older adults to maintain their independence while receiving the necessary medical attention and support. Our multidisciplinary team works to provide a supportive environment through various services including primary care, hospital and emergency care, prescription drugs, vision, dental, and hearing services, as well as physical and occupational therapy.
