For too long, the healthcare system has been a source of frustration for both patients and their doctors. Seniors, in particular, often find themselves lost in a fragmented system, struggling to get the coordinated, primary care they need to stay healthy. At the same time, the primary care physicians they trust are burning out in a structure that prioritizes the volume of services over the value of care. This is the challenge that agilon health was created to solve. Our customers, the primary care physicians, are at the heart of everything we do. We believe that by empowering them, we can create a healthcare system that truly works for their senior patients.

agilon health partners with independent physician groups to help them transition to a value-based care model. This means we provide the necessary tools, technology, and support for physicians to focus on the total health of their patients, rather than just treating sickness. Our model allows doctors to remain independent while benefiting from a network of like-minded peers, sharing best practices and innovative ideas. For patients, this means receiving proactive, coordinated care from a physician who is incentivized to keep them healthy. They are no longer just a number in a fee-for-service system; they are individuals whose long-term well-being is the top priority. We are not just changing how healthcare is delivered; we are restoring the vital patient-physician relationship and building healthier communities, one partnership at a time.
