The journey of CenterWell is a story of strategic evolution, born from the vision of one of America's leading health and well-being companies, Humana Inc. Recognizing a growing need for more integrated and patient-focused healthcare, Humana embarked on a mission to unify its diverse range of payer-agnostic healthcare services under a single, cohesive identity. This vision came to life in March 2021 with the official launch of the CenterWell brand. The new brand was conceived to put patients squarely at the center of their care, a philosophy reflected in its very name and its logo—a blossom with three petals symbolizing the interconnectedness of physical, emotional, and social wellness.

The first services to proudly carry the CenterWell banner were Humana's senior-focused primary care facilities, previously known as Partners in Primary Care and Family Physicians Group. This rebranding was more than just a name change; it signified a reinforced commitment to a value-based care model tailored to the unique needs of seniors. The transformation continued throughout 2022, as other key Humana assets were brought into the fold. The home health division of the newly acquired Kindred at Home was transitioned to become CenterWell Home Health, extending the brand's reach directly into patients' homes. Shortly thereafter, Humana's pharmacy and specialty pharmacy operations were also rebranded as CenterWell Pharmacy and CenterWell Specialty Pharmacy. This strategic consolidation created a powerful, integrated healthcare ecosystem, encompassing senior primary care, home health, and pharmacy services. Today, CenterWell stands as a testament to Humana's forward-thinking approach, operating as the nation's largest provider of senior-focused primary care and a leading provider of home health services, dedicated to delivering accessible, comprehensive, and personalized care to improve the health and happiness of the communities it serves.
