I’m looking for a Clinical or Staff Pharmacist role where I can verify and optimize therapies, lead medication reconciliation and monitoring, support immunizations, and ensure compliance with FDA/DEA/DSCSA while improving outcomes in collaborative teams.

I’m a Registered Pharmacist (California) working as a Staff Pharmacist (Long Term Care; Full Time) and Pharmacist in Charge (PIC) at DosePacker (Premier Pharmacy Services). I verify orders for medications and medical supplies, evaluating appropriateness, safety, clinical/therapeutic efficacy, and cost effectiveness before verification, filling, dispensing, and delivery.

Across my clinical pharmacist roles, I’ve focused on high-alert therapeutic drug monitoring—pharmacokinetic and renal dosing, therapeutic drug monitoring, and frequent drug utilization review. I also lead comprehensive admission and pre-discharge medication reconciliation to support safe transitions of care and reduce interruption of drug therapy.

I’ve compounded parenteral sterile dosage forms under aseptic conditions per USP 797 and ASHP guidance, and I’ve supported antimicrobial stewardship and antibiotic dosing, anticoagulation monitoring, and disease-state management (certified diabetes educator).
