What I'm looking for

I’m looking for a mission-driven health organization where I can deliver evidence-based therapy and coordinated discharge/case management, partner with patients and community agencies, and use my clinical and coding expertise to improve outcomes.

I’m a licensed social worker with over 15 years’ experience in the healthcare industry, building strong relations with providers, clients, and patients. Through claims, coding, and admitting department experience, I’ve acquired hands-on Medicaid and Medicare knowledge, including the policies that guide quality care.

I deliver psychosocial assessments and provide individual and group therapy, with a focus on anxiety and depression using CBT and Talk Therapy. I contribute to treatment team planning, processing commitments, and I own discharge planning and case management services—making referrals and acting as a liaison with referral sources and other hospital departments.

In outpatient and community settings, I manage intakes, assessments, treatment, and discharge planning while maintaining clinical records and ensuring treatment implementation. I also submit monthly service reports as required and monitor/follow up on client needs, collaborating with State, Federal, and local agencies to coordinate services.

I’ve also worked across hospital and community workflows: accountable caseload case management/social work for coordinated care during inpatient and outpatient episodes, admissions support (collecting information and deposits, forms/signatures, scanning documents, and updating ADT systems), and school/home/community-based community social work to reduce stress and strengthen family relationships. Earlier experience as a remote coding specialist sharpened my ability to review and manage ICD-10 diagnostic and treatment coding in medical records.
