At Hospital Prof. Dr. Fernando Fonseca, I support patients and their families across inpatient, outpatient, and emergency care. I assess social situations, identify needs, develop social diagnoses and intervention plans, and coordinate discharge planning.

I work closely with multidisciplinary teams, connect people to specialist services and community resources, and help ensure continuity of social support, including referrals to the RNCCI.

Previously, at the Instituto Português de Oncologia de Lisboa, I provided psychosocial support to oncology patients and family caregivers through crisis, pre-bereavement, bereavement, and post-bereavement situations. I also prepared social discharges, produced social reports, and guided patients and caregivers on their rights.

My experience also includes recruitment, client management, invoicing, corporate partnerships, and organising scientific events and holiday camps. I bring advanced English and Spanish, postgraduate training in palliative care, and a Diploma in Project Management.
