What I'm looking for
I am an experienced and compassionate registered nurse specializing in hospice and palliative care, focused on safe transitions from acute settings to home-based services and inpatient hospice care.
As a Transitional Care Navigator at Journeycare Addus since September 2020, I coordinate hospital transfers, order DME and medications, and manage complex discharges while using Epic, HCHB and Pointcare. I was recognized as Heartland Hospice Champion of the Year in 2015, 2016, 2018 and 2019.
Previously I served as Director of Professional Services and as a case manager/on-call RN, providing clinical oversight, interdisciplinary team leadership, staffing coordination, and education to memory care and assisted living staff. I have managed clinically complex patients including CADD pump setups and high-flow oxygen needs.
I bring strong clinical judgment, medication management, triage and care coordination skills, and I seek roles that allow me to lead patient-centered initiatives, mentor colleagues, and collaborate across disciplines to improve quality and efficiency.
