I am a Master of Social Work–prepared, social worker with progressive experience in behavioral health, care coordination, crisis intervention, and interdisciplinary treatment planning across inpatient, outpatient, and community-based settings. In my current role as PRN Medical Social Worker, I conduct psychosocial assessments, provide crisis intervention, support care planning, and collaborate with interdisciplinary teams to ensure coordinated, patient-centered service delivery.

As a Counselor, I provide individual and group counseling, complete biopsychosocial assessments using standardized tools, participate in treatment team meetings, and maintain accurate clinical documentation in compliance with agency and regulatory standards. Previously, as a Care Coordination Consultant, I developed and coordinated individualized care plans, conducted needs assessments, managed referrals, and collaborated with cross-system partners to address barriers to care and improve service outcomes. I bring demonstrated ability to support recovery-oriented service delivery, monitor treatment planning processes, organize complex workflows, and contribute to quality improvement initiatives.
