I’m a Licensed Practical Nurse with 10+ years of experience in home health and long-term care. I specialize in care coordination, patient follow-up, and medical record review, bringing clarity to complex care plans so patients and providers stay aligned.

At CenterWell Home Health (2021 – Present), I manage a caseload and coordinate care plans by reviewing physician orders and documentation for accuracy, educating patients on treatment plans and next steps, and tracking labs, orders, and results for completion. I also verify insurance, obtain and track prior authorizations, coordinate referrals and testing/specialty care, and communicate with providers and interdisciplinary teams.

Before that, I served as a Charge Nurse at PruittHealth (2017 – 2021), where I supervised nursing staff, managed admissions/discharges/transitions, reviewed and implemented care plans, and ensured documentation compliance. Earlier at Snug Harbor (2015 – 2017), I provided direct patient care while supporting care coordination and maintaining accurate medical records—strengthening the documentation and communication habits I rely on today.
