Katherine Hilario

I’m a Youth Health Educator and School Outreach Liaison at West County Health Center, where I support teen patients and their families with reproductive health education, health coaching, and patient navigation. I counsel youth on reproductive health issues and community resources like health care, food, and housing.

I also manage day-to-day medical documentation by completing daily data entry into medical records systems (eCW and Athena One) to track patient encounters and progress over time. I collaborate with other medical departments to source care for teen patients and provide support through individual meetings and attending visits with medical providers as a patient advocate.

Across school and clinic settings, I educate youth individually and in classroom settings on reproductive health, healthy relationships, mental health and consent, and I research, write, and present lesson plans aligned with the California Healthy Youth Act for grades K-10. I accurately document presentations and activities to support tracking and grant funding, while mentoring youth in individual and group settings.

Earlier in my career, I directed and coached youth and adult sports at City of Santa Rosa Recreation & Parks—creating programs, supervising staff, and building structured learning environments. I’m now expanding my credentials with certifications including Board Certified Patient Advocate, Birth Doula, CPR/BLS Recertification, Wellness Coach, HIV Testing and Counseling, and Pregnancy Testing and Counseling.
