Observe and monitor patients’ behavior, emotional status, and physical well-being, and document daily observations for clinical staff. Support individualized treatment/behavior plans, assist with daily living activities, and de-escalate behavioral crises using approved techniques.

Escort patients to therapy sessions, medical appointments, and activities, and build therapeutic relationships to encourage positive behavior. Record patient progress, incident reports, and shift summaries while coordinating with nurses, therapists, and physicians to support continuity of care.

Maintain documentation of clients’ behavior, progress, and daily activities according to facility standards. Facilitate group activities and complete Medication Administration Record (MAR) documentation, while performing administrative data entry and scheduling tasks to support facility operations.

Provide emotional support and encourage clients while assisting with individualized treatment plans and behavior management strategies. Observe and report changes in clients’ mental or physical condition and help maintain a safe, clean, therapeutic environment in collaboration with the care team.

Support intake, orientation, and discharge processes, perform safety and room checks, and help clients develop coping skills and daily living routines while maintaining confidentiality.
