What I'm looking for
I’m a case management-focused service coordinator with extensive experience supporting children and adults diagnosed with intellectual disabilities. I bring strong organizational skills, confident and empathetic communication, and a steady commitment to safety and well-being.
In my Individual Service Coordinator roles, I developed individualized service plans, completed preadmission screenings and assessments, and maintained HIPAA-compliant documentation and records per DHS ISSA guidelines. I managed caseloads up to 85 individuals, coordinated referrals with community providers, performed in-person monitoring visits, and ensured accurate reporting for abuse and neglect concerns.
Earlier, as a Program Director, I coordinated individual programming and monitored production activities for approximately 75 individuals, supervised Early Intervention, Respite, and SEP programs, and completed grant requests and fiscal reports. I also mentored new hires, conducted audits/reviews by state regulatory agencies, and helped ensure residential providers met CILA regulations.
