At Chardon Woods, I lead social services and interdisciplinary collaboration, coordinating resident-centered care plans, referrals, and support across departments. I manage discharge planning and care transitions, connecting residents and families with appropriate services and community resources.
I coordinate insurance authorizations and support appeals when skilled-service coverage is cut, helping maintain continuity of care. I also apply Medicare and Medicaid requirements to care planning and documentation, advocate for resident rights, and monitor quality measures and chart documentation.
Before Chardon Woods, I directed operations and clinical services at residential treatment and outpatient facilities, including Pinnacle Treatment Center, McIntyre Center, and Axiom Family Counseling. I’m a Licensed Professional Counselor with experience in behavioral health, skilled nursing, and senior living.
