Erin Conlan
@erinconlan
I am a clinical social worker specializing in acute, hospice, and behavioral health care.
What I'm looking for
I am a licensed clinical social worker with broad experience across inpatient psychiatric, hospice, homecare, and community mental health settings. I bring practical clinical training from an MSW program and a history of delivering assessment-driven, person-centered care.
I lead treatment planning, coordinate interdisciplinary care conferences, and complete comprehensive psychosocial assessments from admission through discharge. I provide psychoeducation, supportive counseling, bereavement interventions, crisis assessment, and case management while supervising interns and liaising with community providers.
My work includes acute unit therapy, hospice social work, outpatient and home-based psychotherapy, and emergency screening. I have presented on post-inpatient psychiatric outcomes, completed a thesis on screening approaches for older adults, and contributed to research teams—grounding my practice in evidence and outcomes.
I am comfortable with multiple EMR platforms and clinical documentation systems and am conversationally proficient in Spanish. I collaborate across disciplines, mentor trainees, and seek roles where I can improve continuity of care, support families through transitions, and promote clinically sound, compassionate services.
