What I'm looking for

I’m seeking a long term role in a growing, team oriented professional environment where I can deliver preventative care, strong care coordination/transitional care, and sustainable patient outcomes—while collaborating with coworkers and healthcare professionals to improve results.

I’m a detail oriented Licensed Physical Therapist with 19 years in a broad range of evaluation, treatments and modalities for restoring function to and educating patients. I focus on sustainable patient outcomes, preventative care, medical condition management, life transitions, generating referrals, and rehabilitation management. I bring an excellent ability to establish rapport and trust with patients, their family members, and healthcare professionals.

I’ve delivered care across multiple clinical settings, including senior living and home health. In roles centered on care coordination and transitional care, I identified safety concerns, recommended consistent systems, and supported residents through the continuum of care while reducing chances of unsafe occurrences and hospitalization. I also collaborated with multidisciplinary teams and handled discharge planning.

I’ve balanced direct patient impact with leadership and entrepreneurial experience, including ownership of fitness franchises and as Director in an outpatient physical therapy setting.
