Responded to after-hour phone calls from hospice patients, family members, and facility staff, providing telephonic nursing guidance and triaging calls. Assisted in the completion of weekend admissions by coordinating care, ordering comfort medications, and durable medical equipment.

Assisted individuals dually diagnosed with mental illness and intellectual/developmental disabilities transition out of inpatient psychiatric facilities into less restrictive community settings. Provided a combination of face-to-face and telehealth nursing services, including medication management, health/wellness education, and patient advocacy.

Conducted pre-employment health screenings for new employees, including drug screening, vaccinations, TB testing, and respirator fit testing. Effectively monitored all hospital employees, contractors, volunteers, and physicians, maintaining up-to-date vaccine records and hospital-required competencies.
