At Nightingale Visiting Nurses, I provide skilled nursing and case management for home health patients with complex acute, chronic, and psychosocial needs.

I manage an independent caseload across Start of Care, Resumption of Care, recertification, routine revisits, and discharge. I complete comprehensive assessments, OASIS documentation, medication reconciliation, and individualized care plans.

I work closely with physicians, therapists, caregivers, and interdisciplinary teams to communicate changes in condition, implement orders, and support continuity of care. I also educate patients and caregivers on disease management, medications, fall prevention, infection prevention, and escalation symptoms.

With 10 years of RN experience and prior LPN, residential counseling, and CNA experience, I bring a patient-centered background in clinical documentation, safety, and complex patient care. I use Kinnser/WellSky, Axxess, and Devero home health systems.
