As a chronic care nursing coordinator, I developed individualized, evidence-based care plans, monitored treatment response, and coordinated transitions between healthcare settings. I educate patients on self-management, medication adherence, and lifestyle changes that support better quality of life.

I've also coordinated behavioral health referrals, completed patient assessments, managed electronic health records, and worked with schools on Vanderbilt forms and scoring.
