Role Overview
We are seeking a passionate and experienced Director of Renal Social Work to lead our dedicated team in providing comprehensive psychosocial support to patients with renal disease. In this strategic role, you will oversee the development and implementation of innovative social work programs that enhance patient outcomes and foster a supportive environment for both patients and families.
Responsibilities
- Lead and manage the renal social work team, providing guidance, mentorship, and professional development opportunities to staff members.
- Develop and implement evidence-based social work practices that address the complex needs of patients with renal disease.
- Collaborate with interdisciplinary teams to create patient-centered care plans and facilitate seamless transitions of care.
- Monitor and evaluate program outcomes, ensuring compliance with regulatory standards and quality improvement initiatives.
- Advocate for patients' rights and resources, addressing barriers to care and promoting access to services.
- Conduct training and educational sessions for staff on psychosocial aspects of renal care.
Required and Preferred Qualifications
Required:
- Master's degree in Social Work (MSW) from an accredited program.
- Minimum of 8 years of experience in social work, with at least 5 years in a leadership role within a healthcare setting.
- Current state licensure as a Clinical Social Worker (LCSW) or equivalent.
- Extensive knowledge of renal disease and the associated psychosocial challenges faced by patients.
- Proven track record in program development and management.
Preferred:
- Experience in a renal care setting or similar clinical environment.
- Familiarity with healthcare policy, reimbursement, and regulatory requirements.
