Role Overview
We are seeking an experienced Nephrology Social Work Supervisor to join our dedicated team, where you will play a critical role in enhancing the quality of care for patients with kidney-related conditions. In this position, you will supervise and mentor a team of social workers, ensuring the delivery of comprehensive psychosocial services that align with our mission to improve patient outcomes and support their families.
Responsibilities
- Supervise and support a team of social workers specializing in nephrology, providing guidance on clinical practices and intervention strategies.
- Develop and implement policies and procedures that enhance the psychosocial support available to patients and their families.
- Conduct regular case reviews and provide feedback to team members to foster professional development and ensure high-quality service delivery.
- Collaborate with multidisciplinary teams to create individualized care plans that address the complex needs of patients with kidney disease.
- Monitor and evaluate the effectiveness of social work services and make necessary adjustments to improve patient care.
- Lead training sessions and workshops to educate staff about nephrology-related psychosocial issues and best practices.
Required and Preferred Qualifications
Required:
- Master's degree in Social Work (MSW) from an accredited institution.
- Active state licensure as a Clinical Social Worker (LCSW).
- 5+ years of clinical social work experience, with at least 2 years in a supervisory role.
- Strong knowledge of nephrology and the psychosocial aspects of chronic kidney disease.
Preferred:
- Certification in nephrology social work (CNSW) is highly desirable.
- Experience in a healthcare setting, particularly within a hospital or clinic specializing in nephrology.
- Demonstrated ability to develop and implement educational programs for staff and patients.
Technical Skills and Relevant Technologies
- Proficiency in electronic health record (EHR) systems and documentation practices.
- Familiarity with assessment tools and outcome measures relevant to social work in healthcare.
- Ability to leverage data analytics to assess service effectiveness and patient satisfaction.
