Role Overview
We are seeking a dedicated and detail-oriented Care Coordinator to join our dynamic healthcare team. In this role, you will be pivotal in ensuring seamless communication between patients, healthcare providers, and support services to optimize patient care and outcomes. You will leverage your organizational skills and healthcare knowledge to facilitate care plans, improve patient engagement, and enhance the overall patient experience.
Responsibilities
- Develop and manage individualized care plans for patients, ensuring alignment with their health goals and needs
- Coordinate communication between patients, families, and multidisciplinary teams to facilitate comprehensive care
- Monitor and document patient progress, adjusting care plans as necessary to achieve optimal outcomes
- Educate patients and families about care processes, treatment options, and available resources
- Identify barriers to care and work collaboratively with patients and providers to develop solutions
- Assist in scheduling appointments, follow-ups, and necessary referrals to specialists
- Utilize electronic health records (EHR) to maintain accurate patient information and documentation
Required Qualifications
- Bachelor's degree in Nursing, Social Work, Public Health, or a related field
- 2+ years of experience in care coordination, patient advocacy, or healthcare administration
- Strong understanding of healthcare systems, insurance processes, and community resources
- Excellent communication and interpersonal skills, with the ability to build rapport with diverse patient populations
- Proficiency in using electronic health record systems and other relevant software
Preferred Qualifications
- Experience in a clinical setting or direct patient care
- Certification in care coordination or case management (e.g., CCM, CMC) is a plus
- Familiarity with population health management and quality improvement initiatives
- Knowledge of regulatory requirements and best practices in patient care coordination

- Proficient in electronic health record (EHR) systems, telehealth platforms, and care management software
- Strong analytical skills to assess patient data and identify trends
- Ability to navigate and utilize various healthcare databases and patient management tools
