Role Overview
We are seeking a dedicated and detail-oriented Care Coordinator to enhance patient care by facilitating communication between patients, families, and healthcare providers. This role is critical in ensuring that our patients receive the appropriate resources and support necessary for their health journey.
Responsibilities
- Act as a liaison between patients and healthcare providers, ensuring clear communication and understanding of care plans
- Coordinate patient appointments, follow-ups, and referrals to specialists as necessary
- Assist patients in navigating healthcare systems and accessing community resources
- Monitor patient progress and provide ongoing support to address any barriers to care
- Document and maintain accurate records of patient interactions and care plans
- Participate in interdisciplinary team meetings to collaboratively discuss patient needs and treatment plans
Required and Preferred Qualifications
Required:
- Associate's degree in healthcare, social work, or a related field
- 2+ years of experience in a healthcare setting, preferably in care coordination or patient advocacy
- Strong understanding of healthcare systems and community resources
- Excellent communication and interpersonal skills, with a compassionate approach to patient care
Preferred:
- Bachelor's degree in nursing, social work, or a related field
- Experience with electronic health records (EHR) systems and patient management software
- Knowledge of medical terminology and healthcare regulations
Technical Skills and Relevant Technologies
- Proficiency in using EHR systems and telehealth platforms
- Ability to analyze patient data and generate reports for quality improvement
- Familiarity with project management tools to coordinate patient care initiatives
