Role Overview
We are seeking an experienced Advanced Practice Gerontological Nurse Practitioner to join our innovative healthcare team. In this role, you will provide expert clinical care to the aging population, focusing on enhancing their quality of life through comprehensive assessments, advanced clinical interventions, and collaborative care planning.
Responsibilities
- Conduct thorough health assessments and develop individualized care plans for older adults, incorporating evidence-based practices.
- Monitor and manage complex chronic conditions, ensuring optimal health outcomes through proactive interventions and patient education.
- Collaborate with interdisciplinary teams to coordinate patient care, including referrals to specialists and community resources.
- Provide mentorship and guidance to junior nursing staff and students, fostering a culture of continuous learning and professional development.
- Engage in health promotion activities and community outreach to educate patients and families about geriatric health issues.
Required and Preferred Qualifications
Required:
- Master's degree in Nursing with a specialization in Gerontology or related field.
- Current Nurse Practitioner licensure in the state of practice and certification as a Gerontological Nurse Practitioner.
- Minimum of 3 years of clinical experience in geriatric care, with a strong understanding of age-related health issues.
- Proven ability to perform comprehensive assessments and develop effective care plans.
Preferred:
- Experience in a remote healthcare environment, utilizing telehealth platforms.
- Familiarity with electronic health records (EHR) systems and data management.
- Advanced certifications in gerontological nursing or palliative care.
Technical Skills and Relevant Technologies
