Role Overview
We are seeking a dedicated and experienced Lead Home Health Attendant to join our team in delivering exceptional care to our clients in a fully remote capacity. In this role, you will be responsible for overseeing the care provided by our home health attendants, ensuring that all clients receive the highest level of support tailored to their individual needs.
Responsibilities
- Supervise and coordinate the activities of home health attendants to ensure compliance with care plans and quality standards.
- Conduct regular assessments of client needs and preferences, updating care plans accordingly.
- Provide training and mentorship to home health attendants, fostering professional development and adherence to best practices in care.
- Act as the primary point of contact for clients and their families, addressing concerns and ensuring satisfaction with care services.
- Monitor and evaluate the effectiveness of care strategies, making adjustments as necessary to improve client outcomes.
- Collaborate with healthcare professionals, including nurses and physicians, to coordinate comprehensive care for clients.
Required and Preferred Qualifications
Required:
- Minimum of 3 years of experience in home health care or a related field, with at least 1 year in a supervisory role.
- Strong knowledge of home health care regulations and best practices.
- Excellent communication skills, both verbal and written.
- Compassionate and patient-centered approach to care.
Preferred:
- Certification in home health care or nursing.
- Experience with electronic health records (EHR) systems.
- Knowledge of chronic disease management and geriatric care.
Technical Skills and Relevant Technologies
- Proficient in using telehealth platforms and remote monitoring technologies.
- Familiarity with care management software for tracking client progress and documentation.
