Role Overview
We are looking for a passionate Habilitation Specialist to join our interdisciplinary team. In this role, you will be responsible for designing and implementing individualized habilitation plans that promote the development and enhancement of skills necessary for daily living and community engagement for individuals with disabilities.
Responsibilities
- Conduct comprehensive assessments to identify individual needs and strengths, developing tailored habilitation plans that align with client goals.
- Implement, monitor, and adjust habilitation strategies in collaboration with clients, families, and other professionals to ensure optimal outcomes.
- Provide direct support and training in social, communication, and adaptive skills, fostering independence and self-advocacy.
- Document progress and outcomes meticulously, utilizing data-driven approaches to inform ongoing treatment adjustments.
- Engage with community resources and stakeholders to facilitate access to services and enhance client quality of life.
- Support and train staff and families on habilitation principles and strategies to create a cohesive support system for clients.
Required and Preferred Qualifications
Required:
- Master's degree in Social Work, Psychology, Rehabilitation Counseling, or a related field.
- Current licensure or certification relevant to habilitation services.
- Experience working with individuals with disabilities, demonstrating a deep understanding of their unique challenges and strengths.
- Strong analytical and problem-solving skills to evaluate client needs and adapt strategies effectively.
Preferred:
- 2+ years of experience in a direct support role within habilitation or related services.
- Familiarity with evidence-based practices in habilitation and behavior support.
- Experience collaborating with interdisciplinary teams to enhance service delivery.
Technical Skills and Relevant Technologies
- Proficiency in data collection and analysis tools to measure client progress.
- Familiarity with assistive technology and adaptive strategies that enhance client communication and independence.
- Understanding of electronic health record (EHR) systems for documentation and reporting.
