Role Overview
We are looking for a skilled Lead Case Manager to oversee and enhance our case management processes. In this pivotal role, you will drive strategic initiatives that improve client outcomes, ensuring that best practices are implemented across all case management activities. Your expertise in navigating complex client needs will be essential in fostering a supportive environment for both clients and team members.
Responsibilities
- Lead and mentor a team of case managers, providing guidance on best practices and case management methodologies.
- Develop and implement case management protocols that align with organizational goals and compliance standards.
- Conduct regular assessments of client needs and service delivery effectiveness, adjusting strategies as necessary.
- Facilitate interdisciplinary collaboration to ensure comprehensive support for clients.
- Monitor case management outcomes and generate reports to inform stakeholders of progress and areas for improvement.
- Act as the primary liaison between clients, service providers, and external agencies to ensure seamless service delivery.
Required and Preferred Qualifications
Required:
- Bachelor’s degree in social work, psychology, or a related field.
- 5+ years of case management experience, with a proven track record in a leadership role.
- Strong understanding of relevant regulations and compliance standards in case management.
- Exceptional organizational skills and the ability to manage multiple priorities effectively.
Preferred:
- Master’s degree in a relevant field.
- Experience with data analysis and reporting tools to track case management outcomes.
- Certification in case management (CCM or ACM) is a plus.
Technical Skills and Relevant Technologies
- Proficient in case management software and electronic health records (EHR) systems.
- Experience with data management tools to analyze client data and track progress.
- Familiarity with telehealth platforms and remote service delivery methods.
