Sevita provides community-based services for individuals with intellectual and developmental disabilities. The Referral Intake Coordinator assists clients in making the first step towards their better future.
Requirements
- Provide compassionate customer service and establish/maintain relationships with referral sources
- Provide knowledgeable information regarding company services including programs and product lines offered
- Answer calls and process email, fax, website and internal inquiries in a timely and professional manner
- Gather and enter necessary data in the intake system
- Ensure supporting documentation is entered into the Electronic Health Record
- Provide accurate information to person served
- Facilitate determination of referral viability
- Establish and maintain knowledge and proficiency of health plans
- Verify benefits for health insurance using portals
- Identify all types of funding possibilities
- Routinely run and analyze reports in regards to productivity
- Collaborate and communicate issues and concerns with all internal and external staff
- Assist with special projects as department growth occurs within additional markets
- Perform other related duties and activities as required
