Optimize Health

Optimize Health was initially launched in 2015 in Seattle, Washington, to address gaps in care between in-person visits. Recognizing the impact our technology and clinical services had on practices and their patients, we expanded our offerings to specialize in remote care with monitoring and disease management programs. We recognized that chronic condition patients had to track their vitals at home, which they often forgot. As a result, many preventable hospitalizations or emergency care occurred. Providers had blind spots in understanding how their patients were doing outside their exam rooms.

These challenges in healthcare were the catalyst for the birth of Optimize Health. We deliver technology that enables more visibility between office visits, empowers patients to be more engaged in their health, and supports physicians managing chronic conditions beyond practice walls.
