The Social and Health Equity Navigator ensures that Molina’s programs, policies, and processes integrate a cultural humility lens and acknowledge the historical and institutional social inequalities that contribute to poor health outcomes among members of groups based on race or ethnicity, gender identity, sexual orientation, age, disability, socioeconomic status, mental health, and geographic location, among other factors. The Social and Health Equity Navigator is a member of the Health Equity Team, and supports the Molina Health Equity Plan by bringing best programing practices. These include community health disparities assessment, data collection, gap analysis, and program development, implementation, and evaluation of initiatives. Social and Health Equity Navigators have diverse backgrounds and lived experiences and come from the geographic communities that Molina serves. This position supports Molina’s overall health equity strategy and collaborates with other Molina departments including Population Health Management, Quality, and Community Engagement among others, to build infrastructure and programming to ensure an equitable approach to providing healthcare services.
This is a remote position with up to 70% travel by car throughout the service delivery area that you are responsible for. The qualified candidate MUST be located in CLARK COUNTY, NEVADA. A clean DMV driving record, proof of auto insurance, and reliable transportation is required.
This is a full time position, 40 hours per week and may require some evenings, weekends, and holidays.

Job Summary The Social and Health Equity Navigator will assist members in addressing social conditions that impact health outcomes, assist with the development of a process to collect, analyze, and report data metrics, and ensure these cost-effective services are provided to members who need them the most. The Social and Health Equity Navigator will also work collaboratively with other departments to identify population SDOH needs and work to help find solutions via partnerships with community organizations or other agencies. Bachelor’s Degree in public health, social work or behavioral sciences or an equivalent education and demonstrated experience in the social determinants of health Master’s Degree in public health, social work or behavioral sciences or similar discipline Licensed in Social Work, Counseling, or other related field Working environment is generally favorable and lighting and temperature are adequate. Work is generally performed in an office environment in which there is only minimal exposure to unpleasant and/or hazardous working conditions. Must have the ability to sit for long periods.
