Innovative Integrative Health

Innovative Integrated Health (IIH) is a customer-centric organization dedicated to serving the elderly and enabling them to live with dignity and independence in their own homes. Many seniors and their families face the challenge of managing chronic illnesses and disabilities, often leading to the difficult decision of moving into a nursing home. IIH addresses this by providing a comprehensive alternative through the Program of All-Inclusive Care for the Elderly (PACE). This unique model of care is designed for individuals aged 55 and over who require a nursing home level of care but can safely live in the community with the support of PACE services. Our customers, who are often from frail, underserved, and multi-ethnic communities, receive a personalized and coordinated care plan that addresses their complete well-being.

Our approach is built on the understanding that our participants' needs extend beyond just medical treatment. That's why we offer a holistic suite of services that encompasses not only primary and specialty medical care, but also social services, nutritional guidance, transportation, and assistance with daily activities. We recognize the importance of cultural and linguistic diversity, and our highly-trained, multi-lingual staff ensures that care is delivered in a way that is respectful and understandable to each individual.
