The Licensed Clinical Social Worker conducts psychosocial evaluations, provides individual and group psychotherapy, care management services and discharge planning. Develops and implements patient treatment plans and participates in treatment team conferences. Relies on education, experience and judgment to accomplish job. Works under supervision required by regulation and licensing board.
Clinical Practice and Care Management
- Serves as the case manager for assigned participants. Coordinates the interdisciplinary assessment process.
- Completes psychosocial assessment within guidelines defined by the organization.
- Conducts group and individual counseling therapy and/or case management activities as appropriate to the treatment plan, and in the guidelines of her LCSW credentials.
- Conducts family meetings as necessary and as directed.
- Develops and implements high quality patient treatment plans based on thorough participant assessment and evaluation, analysis of family and home dynamics, and assessment of participant's capacity and potential to cope with problems of daily living in an effort to promote the high-quality health and wellness services provided by the department.
- Follows established practice guidelines with regard to work and participant scheduling and demonstrates flexibility in scheduling to meet practice and participant needs.
- Demonstrates professional judgment in reporting cases of suspected abuse or neglect while maintaining participant and family member confidentiality in an effort toward ensuring the provision of high-quality health and wellness services by the department.
- Responsible for Encounter Data reporting if/when applicable. The social worker must be LCSW in order to add and verify all social work encounters for revenue base with CMS and other applicable payers.
Documentation
- Completes treatment plan documentation and updates according to participant needs and organizational expectations.
- Completes all documentation as required for all participant and caregiver counseling sessions, according to CMS and/or other applicable payer guidelines, in order to successfully capture encounter data reporting.
- Completes documentation of all participant and caregiver interactions.
- Completes documentation of all group, individual and family activities.
- Explains and discusses the living will and durable power of attorney to participants and family members in a sensitive and caring manner in accordance with established policy and procedure.
- Follows policies and procedures regarding discharge planning, evaluations, chart documentation, and follow-up coordination, ensures appropriate and efficient completion of authorization to provide or receive information.

- Works collaboratively to solve problems, improves processes, develop services, and acts as a participant advocate.
- Participates in departmental and organizational quality improvement initiatives.
- Actively participates in the development and implementation of the interdisciplinary treatment plan. Responsible for the active care plan of each individual assigned.

- One year experience working in Case/Care Management

- Master's Degree
