Nutritional Support Coordinator (Remote – US)


Job Title: Nutritional Support Coordinator
Compensation: $17.00 per hour - $22.00 per hour. (based on experience)
Location: Remote (US)
Work Environment: Remote with HIPAA-compliant home office setup

What We’re Looking For:

We’re looking for a knowledgeable, patient-centered Nutritional Support Coordinator to manage patients’ nutritional care through remote counseling and care coordination. The ideal candidate has strong clinical nutrition expertise, excels at simplifying complex dietary concepts for diverse populations, and is passionate about helping patients improve their health through evidence-based nutritional guidance. You thrive in fast-paced environments, build rapport easily with patients, and collaborate effectively with clinical teams.


What You’ll Do:

Patient Outreach & Nutritional Counseling
  • Manage an assigned panel of 300-350 patients and conduct required monthly nutritional outreach and support.
  • Conduct 50-60+ daily phone calls with patients, providing nutritional care plan support and health coaching tailored to their specific conditions.
  • Maintain call quality standards and professional communication in every patient interaction; pass regular QA call reviews.
Nutrition Planning & Patient Education
  • Develop and implement personalized nutritional care plans based on patient assessment and clinical needs.
  • Educate patients on nutrition concepts, dietary modifications, and weight management strategies.
  • Simplify complex nutritional information for diverse patient populations.
Care Coordination & Collaboration
  • Collaborate with clinical teams, providers, and caregivers to identify and address nutritional and clinical needs.
  • Support care coordination goals by staying on schedule with monthly outreach targets.
  • Partner closely with the clinical team to support comprehensive patient care.
Documentation & Compliance
  • Document all patient interactions in real time using the care coordination platform.
  • Maintain HIPAA compliance in all patient communications and documentation.
  • Ensure accurate documentation to support care coordination goals.


Required Qualifications:
  • Bachelor’s degree in Dietetics or Nutrition.
  • Minimum 2+ years of clinical nutrition counseling or dietary care coordination experience.
  • Experience supporting patients with chronic conditions in remote, in-home, or in-facility settings.
  • 2+ years of Electronic Medical Records (EMR) and computer navigation experience.
  • Demonstrated ability to educate patients and simplify complex nutritional concepts.
  • HIPAA-compliant home office setup with minimum 50 Mbps download / 5 Mbps upload internet capability.
  • Must be legally authorized to work in the United States.


Preferred Qualifications:
  • Experience in remote patient care or telehealth nutrition counseling.
  • Experience with care management or chronic disease management programs.
  • Familiarity with weight management or behavioral health principles.
  • Previous experience managing large patient panels (200+).
  • Bilingual capability (English and Spanish preferred).

  • 100% Remote with a Monday–Friday schedule—no weekends or late nights.
