I’m a Certified Professional Risk Adjustment Coder (CRC & CPC) with 3+ years of experience, focused on accuracy in HCC (Risk Adjustment) coding. I work to ensure diagnoses are supported, specific, and compliant with CMS risk adjustment guidelines.

Currently, I’m working as an HCC (Risk Adjustment) coder at Omega healthcare (March 2026 – Till date). Previously, I worked as an HCC coder at Claxon Healthcare Pvt. Ltd. (Sept 2022 – Dec 2025), gaining strong experience across Medicaid and Medicare populations.

I conduct retrospective and concurrent HCC chart reviews using ICD-10-CM guidelines, coding chronic and active diagnoses supported by MEAT criteria. I identify unsupported, missing, or invalid HCC conditions, and I support the process by accepting or rejecting HCCs based on documentation quality.

I’m detail-oriented and dedicated to outpatient encounter review—reviewing provider documentation for diagnosis accuracy and specificity. My background includes chronic condition capture, outpatient documentation standards, and EMR/HER systems work that strengthens consistency across workflows.
