What I'm looking for

I’m seeking a remote Care Coordinator role to support TRICARE beneficiaries, using my triage, de-escalation, and care-transition expertise. I’ll coordinate across teams and document accurately in EHRs to keep care flowing smoothly.

I’m a Licensed Practical Nurse with 13+ years at the VA, currently serving as a Lead Screener (GS-7). I triage thousands of patients and staff daily, determine whether care is emergency, clinical, or deferred, and coordinate transitions across emergency, inpatient, and outpatient settings.

I’m known for de-escalation and crisis intervention, plus remote-ready communication to assess needs, stabilize situations, and keep teams aligned. I document in CPRS/VistA under federal compliance standards, manage daily screening-team operations, and train/validate competencies for new clinical staff.
