Susan Gagan

I’m a Compassionate Licensed Practical Nurse with experience in patient triage, care coordination, and electronic medical documentation. I focus on phone-based patient assessment, intake interviews, and clear clinical documentation that supports timely decisions.

In my roles, I’ve conducted telephone triage for patients by collecting medical data and identifying health concerns using clinical guidelines. I provide patient education and health guidance based on symptoms and physician protocols, helping patients understand next steps.

I also perform physical and psychosocial assessments to determine patient needs and measure and document vital signs and patient conditions. When changes or emergencies arise, I respond promptly, implement care interventions, and communicate updates to physicians and nursing leadership.

Beyond direct patient care, I’ve supervised and directed daily assignments for resident care assistants and coordinated care plans with healthcare providers and family members. I bring strong interpersonal and communication skills, with additional leadership experience in training and team management.
