Rachael Hartley

I am a Family Nurse Practitioner with a Master of Science in Nursing (Family Nurse Practitioner, December 2018) from Indiana Wesleyan University, a Bachelor of Science in Nursing (May 2012) from Ball State University, and a Bachelor of Science in Speech Pathology (May 2008). I am highly motivated and committed to developing strong patient relationships and improving population health.

My clinical experience spans emergency and urgent care as a Nurse Practitioner (Riverview Hospital Emergency Department, 2023–current; Community Health Network MedCheck, 2019–2023; PRN Nurse Practitioner current) and prior Registered Nurse roles in emergency and intensive care settings (Community Hospital Anderson Emergency Room, 2012–July 2017; Intensive Care Unit, 2017–2018). I also served as a SANE at the Madison County Sexual Assault Treatment Center (2013–2022), which strengthened my forensic and trauma-informed care skills.

I hold active licensure and certifications including Registered Nurse, State of Indiana; Nurse Practitioner, State of Indiana; BLS, ACLS, and PALS (American Heart Association). I seek to apply my acute care, critical care, and population-health experience in a collaborative setting where I can continue building therapeutic relationships and deliver measurable improvements in patient outcomes.
