Conduct diagnostic evaluations to determine eligibility for outpatient mental health services and develop and update treatment and crisis plans. Provide weekly individual and/or family therapy, complete clinical documentation, and facilitate DBT and art therapy groups while supporting clients through crisis assessment and discharge planning.

Screen and assess homeless veterans for program eligibility and enroll qualifying clients into the Homeless Veteran Reintegration Program (HVRP). Build individualized employment plans, provide ongoing assessment and support, coordinate services with program leadership, and conduct outreach to generate referrals and placement outcomes.

Perform face-to-face biopsychosocial assessments for clients with co-occurring disorders and develop service coordination plans tied to ongoing medical needs. Provide referrals and ongoing monitoring of service effectiveness, facilitate counseling and support groups, maintain DAP notes, and submit monthly statistical reports.

Schedule and deliver weekly individual and couples therapy sessions, supporting clients with issue identification and therapeutic problem-solving. Provide external referrals (e.g., AA/NA), participate in case conferences with a medical case manager, and prepare clients for discharge from counseling services.

Monitor and support patient safety and status for drug and alcohol clients, including vital sign documentation and UDS on return from pass. Conduct mandatory searches and suicide assessments on admission, document behaviors/goals/interventions with DAP notes, facilitate 12-step model groups, and mediate client conflicts.

Observe and monitor client safety and status, distribute medications, and provide hands-on physical care including hygiene, meal preparation, and chores. Document client behaviors accurately and consistently and escort clients to medical appointments and community activities.

Support clinical workflows by greeting and escorting patients, recording vital signs and medical history in EPIC, and preparing patients for physician procedures. Assist with lip biopsy and myringotomy procedures, maintain instrument sterilization and specimen handling, and support scheduling and room preparation.

Coordinate unit operations by composing and updating monitor room assignments and maintaining whiteboard status for staff and patient needs. Enter dialysis, meal, and intake/output information in Smart Chart, create and manage admission/discharge charts in Invision, and document and communicate critical test results.

Recruited, hired, trained, supervised, and evaluated part-time student staff for the Sansom Place East/West Emergency Call Center. Managed move-in/move-out operations and building security access, and produced monthly service reports while coordinating staff coverage.
