What I'm looking for

I’m seeking a remote or hybrid role in care coordination, case management, or telehealth—leveraging my experience in patient education, documentation, and care planning to ensure clear communication, continuity of care, and patient advocacy.

I’m a Registered Practical Nurse with diverse experience in clinical care, patient coordination, and interdisciplinary collaboration. My nursing experience has ranged from long-term care to community settings to an interventional pain clinic. I’m recognized for strong clinical judgment, patient advocacy, and clear communication, with a focus on positive patient outcomes.

I currently work as a Flurscopy Nurse(2023–present), where I partner with physicians to perform X-ray and ultrasound-guided procedures for pain management. I work closely with interdiciplinary staff to support care planning, conduct pre- and post-procedure assessments, and document accordingly.

Previously, I provided comprehensive nursing care for residents with complex needs at Woodlands of Sunset (LTC) (2022–2023), and supported clients in-home with ADL assistance, mobility support, and medication reminders through Right at Home (2022), strengthening continuity of care and safety.
