What I'm looking for

I’m looking for an RN role where my education and case management experience strengthen safe, effective transitions—especially for CKD/ESRD—through quality-driven documentation, proactive outreach, patient education, and collaboration to improve outcomes.

I’m a Registered Nurse (BSN) focused on optimal patient care through strong clinical judgment, proactive outreach, and thorough coordination across settings. I seek a role where my education, training, and case management skills directly improve outcomes.

In my current position as a Telephonic Transition Case Manager / RN Care Manager at Strive Health, I support patients with complex Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD) as they transition between care settings. I apply the Coleman Model of transitional care, ensure compliance with NCQA accreditation standards through accurate documentation, and help minimize readmissions.

Before that, I served as a Telephonic Integrated Care Nurse with DaVita Village Health Integrated Kidney Care and as a Telephonic Nurse Case Manager with Aetna (CVS Health) and Allegiance Care Management (Cigna). I telephonically assess and coordinate medical and psychosocial needs, manage diverse patient panels, provide culturally sensitive education, and communicate clear updates to patients, families, and interdisciplinary teams.

Earlier in my career, I delivered acute and subacute dialysis nursing in hospital and clinic settings, including hemodialysis care, patient/family education, and vascular access monitoring. I also worked in home infusion nursing and held charge nurse/supervisor responsibilities, bringing a steady leadership mindset and a patient-advocacy approach to every transition.
