I've delivered high-acuity medical oncology care at Stony Brook University Hospital and coordinated complex long-term care remotely at Centers Plan for Healthy Living.
As a Remote Nurse Care Manager, I managed monthly caseloads of 120–125 MLTC members, conducted telephonic evaluations, developed individualized care plans, and coordinated aide services, therapies, medical supplies, referrals, and authorizations. I partnered with hospital teams, primary care providers, specialists, caregivers, and utilization management teams to support safe discharges, medication reconciliation, and care-plan goals.
At Stony Brook University Hospital, I provided direct care for medical oncology patients, including chemotherapy and immunotherapy administration, central-line management, IV therapy, blood transfusions, enteral and parenteral nutrition, wound care, and tracheostomy care.
