Role Overview
We are looking for a dedicated and detail-oriented Care Coordinator to join our dynamic healthcare team. In this role, you will play a vital part in managing patient care processes, ensuring seamless communication among patients, families, and healthcare providers, and facilitating access to appropriate services. Responsibilities

- Facilitate referrals to specialists and other healthcare services, ensuring that patients receive timely access to necessary care.
- Monitor and track patient progress throughout their care journey, providing updates to healthcare teams and adjusting care plans as needed.
- Educate patients and families on care processes, treatment options, and available resources to empower informed decision-making.
- Maintain accurate and up-to-date patient records in compliance with healthcare regulations and organizational policies.
Required and Preferred Qualifications
Required:
- Bachelor's degree in Healthcare Administration, Nursing, Social Work, or a related field.
- 2+ years of experience in care coordination or a similar role within a healthcare setting.
- Strong understanding of healthcare systems, processes, and patient advocacy.
- Proficient in electronic health records (EHR) systems and Microsoft Office Suite.
- Excellent verbal and written communication skills, with the ability to interact effectively with diverse populations.
Preferred:
- Certification in Care Coordination or Case Management (e.g., CCM, CMC).
- Experience with telehealth services and remote patient monitoring technologies.
- Familiarity with community resources and services available to patients.
Technical Skills and Relevant Technologies
- Proficient in using electronic health record (EHR) systems.
